The SOAP Notes section of the Administration menu is where you can customize the default SOAP Note template used across all clients for your organization.

Click on the Administration tab in the Catalyst portal. Then, click on SOAP Notes in the Administration Menu to open the SOAP Note Template Configuration page.



Every SOAP Note template has six default fields that cannot be deleted. These fields are: date, start time, end time, location, notes, and signature.

These fields will auto-fill in the app with today's date, the time at which the student notebook was opened for start time, the current time for end time, and your exact GPS coordinates for location (you must allow Catalyst access to your location in the device settings for location to be detected).


To add custom fields, click Add New Entry at the bottom of the page.

  

The New Form Entry window appears. Users can pick from three types of fields: QuestionText, and Section Break.


Questions

Questions act as fields for collecting information from the user completing the SOAP Note Response. 

Upon selecting Question as your Field Type, there are eleven Answer Types that each collect data in a different way. 

Answer Types

Free Text
Free Text is an open text field in which users can type using their device keyboard. 
'Entry Text' will appear directly above the open text field (e.g. "Type your subjective observations in the below text box.").
'Maximum Length' will impose a character limit on the text box. e.g. "1500"

Likert Scale
A likert scale is used to measure the users preferences or degree of agreement with a statement.
'Entry Text' will appear directly above the likert scale. (e.g. "Indicate how you feel about the following statement: The student was cooperative." ).

'Comma-Separated Choices' will dictate the options on the likert scale. Enter three to seven options, separated by commas, in the order you wish for them to appear from left to right. e.g. "Strongly Disagree, Disagree, Neutral, Agree, Strongly Agree"

Email
A text field specifically for e-mail addresses. 
'Entry Text' will appear directly above the text field (e.g. "Enter the email address of the client's parent/guardian.").

Numeric
A text field specifically for numbers. 
'Entry Text' will appear directly above the field (e.g. "What is the temperature outside in degrees Fahrenheit?" ).
'Minimum Number' and 'Maximum Number' will restrict the range of numbers that are accepted by the form (e.g. Minimum Number = 0 and Maximum Number = 105)

Date Picker
A spinning picker that includes month, date, and year. 
'Entry Text' will appear directly above the date picker (e.g. "What is the date of the clients next scheduled session?").

Time Picker
A spinning picker that includes hour, minute, and am/pm. 
'Entry Text' will appear directly above the time picker (e.g. "What is the time of the client's next scheduled session?").

Duration
A spinning picker that includes minutes and seconds. 
'Entry Text' will appear directly above the time picker (e.g. "Was the client late to the session? If so, by how many minutes and seconds?"). 

Signature
A signature box which can be signed using a finger or stylus. 
'Entry Text' will appear directly above the signature box (e.g. "Signature of the Parent/Guardian").

Slider
A circle that can be dragged along a sliding scale. 
'Entry Text' will appear directly above the slider (e.g. "On a scale of 1 to 10, with one being poor and ten being outstanding, how do you feel the student is responding to treatment?"). 
'Minimum Number' is the numeral selected when the slider is dragged completely to the left (e.g. 1).
'Maximum Number' is the numeral selected when the slider is dragged completely to the right (e.g. 10).

Multiple Choice
A spinning picker that includes options of your choosing. 
'Entry Text' will appear directly above the spinning picker (e.g. "How many sessions does this student have per week, on average?").
'Comma-Separated Choices'  will dictate the options available to the user to select. e.g. "One, Two, Three, Four, Five, Six" 

Multiple Select
Like multiple choice, but more than one option can be selected. 
'Entry Text' will appear directly above the multiple select box (e.g. "Please select all that apply. There is a recent change to this client's:").
'Comma-Separated Choices' will dictate the options in the multiple select box (e.g. "Home Address, Contact Phone Number, Medications, Insurance Policy, There Are No Changes").

Required
Is Required? is an optional checkbox for all answer types. Checking this box makes a field required for submission. 

Validation 
If the question is required but was not completed upon saving of the SOAP Note Response, the user will receive a Validation Failed message. Missed questions are indicated by a red border. 

 

Section Break

Section Breaks are centered, bolded, and underlined to act as headers in your SOAP Note Template. There is no place for users to input data, section breaks are simply text. 

Text

Text is similar to a section break but it is left aligned and unbolded. There is no place for users to input data, it simply consists of text. 

 

Preview

As you are building your SOAP Note Template, preview how it will appear on a device on the right hand side of the page.


Drag and Drop

Rearrange the order of the fields by hovering over the line and dragging to a new position. 


Delete and Edit Fields

Use the Actions menu on the right to Delete and/or Edit the fields. 


Please Note

To maintain historical consistency and quality of SOAP Notes responses, only the required checkbox and minimum and maxiumum values can be edited. Entry text and comma seperated choices cannot be edited once the question is saved. If you must edit the wording or available options for a question, delete the field and re-enter your revised version via 'Add New Entry'. 


It Is Automatically Published

The SOAP Note Template is automatically published and available across your organization as changes are made to it. There is no save or publish button. 

 

It Is Organization-Wide

The SOAP Note Template will serve as the uniform template for all students and users among your organization.
